One entry per Member. Members signing up for e-statements between July 7 and August 11, 2003, will be automatically entered in the prize drawing. Drawings will be held July 15, July 22, July 29, August 5 and August 12, 2003. Contest is open to all Mazuma Members in good standing. Signing up for e-statements is not required for prize drawing. Contest is void where prohibited. Odds of winning depend on the number of entries received. Individuals must be 18 or older to enter contest. Winning Members will receive $100 to be placed in the account(s) of their choice. The winnings will be placed in winning Members accounts by August 31, 2003. Board members, supervisory committee members, employees of Mazuma Credit Union, its affiliates and their immediate family members are excluded. The winning Members will be notified by phone, e-mail or mail. Prize is non-transferable. The winning Members grant Mazuma Credit Union the right to use their names and photographs for promotional purposes. Members not signing up for e-statements should mail a 3x5 card including name, address, home phone, work phone and Mazuma account number to: Mazuma Credit Union / Attn. Marketing / 9300 Troost Avenue, Suite 1200 / Kansas City, MO 64131-3000 before August 11, 2003. To receive the name of the winning Members, send your request with a self-addressed, stamped envelope to the above address. All requests must be received by August 1, 2003. By accepting the prize, winning Members agree that the prize is awarded on the condition that Mazuma Credit Union and its agents, representatives and employees will have no liability whatsoever for any injuries, losses or damages of any kind resulting from acceptance, possession or use of the prize. Winning Members must be responsible for any tax consequences, if applicable.
