SWITCH YOUR CHECKING TO MAZUMA CREDIT UNION

	I would like to switch from my current financial institution and apply for a Mazuma checking account.
	I currently have a Mazuma checking account and would like to switch my direct deposits and automatic payments, then close my old account.
